The plot is set in the 1940's in a village of Travancore, British India. Kaliyappan, the last hangman of Travancore dynasty is dragging his remaining life by consuming alcohol and worshipping the Mother Goddess. Jeevan Mashai is the last in the line of a family of Ayurvedic physicians who run the 'Arogya Niketan' treatment center in the village. The gifted Jeevan, who does his treatment through 'Nadi pariksha' or pulse examination, can feel the pulse of people & diagnose their malady, its severity & its fatality. Simultaneously the subject of respect & anger, Mashai faces a skeptic in the form of allopathic doctor Pradyut (Vijayaraghavan) who is convinced that Mashai is a quack & is manipulating people.

Neighbors & childhood friends Appukuttan (Byon) & Thanka (Ashwati Lal) grow up together falling in love. However, they face stiff opposition from Appukuttan's family on account of Thanka's family being poor. Forced to leave Appukuttan, Thanka decamps with local moonshiner Velu (Thirumurugan), who had an eye on Thanka, but whom she hated, to a far off place. However, Appukuttan & Thanka continues to pine for each other.

SP Hameem Hyder (Indrajith), a cop investigating a series of 2 murders which seemed to be related, has an encounter with the killer as he tries to stop the 3rd one, but which leaves Hyder incapacitated for months. His unfinished investigation becomes the focus of a reality show called '3 Eye' where a priest Fr. Varghese Punyalan (Joy Mathew) confesses to the murders. However, Hyder, also invited to the show, refuses to accept the priest's confession & tries to solve the murder on live TV.

Bangalore-based professionals & friends Balu (Fahadh Faasil) & Isha (Radhika Apte) fall in love with each other, get married & move to Kochi, but a fight between them causes Isha to fall out of love & they head for divorce with Balu continuing to question why. In between this, Balu happens to save a girl, Ameena (Rajshri Deshpande), from a murder attempt & befriends the ebullient radio jockey & activist Gargi (Sagarika Bhatia).

Raman (Dileep) returns to his hometown of Ramapuram in Kerala after 30 years to encash an investment his late parents had left for him. Unbeknownst to Raman, he was the son of Veebhadran (Santhosh Keezhattoor), who had killed Indrasimhan (Anand) of Ravanapuram, Ramapuram's rival village. Raman ends up in the house of Indrasimhan's brother Narasimham (Nagineedu), who had vowed to kill him. Raman tries to stay alive, while Narasimhan's daughter Krishna (Nikki Galrani) thinks Raman is chasing her.
