Tommy Heller has been locked away in a mental institution for the past twenty-five years after committing the Camp Murder Massacre. When his careless orderly lets him escape from the care of Doctor Lewis, he returns to the Camp to continue his murder spree.

A mafia drug lord has unleashed a new, highly addictive stimulant on the streets called HT25, derived from sharks held captive in a secret lab, and which causes monstrous side effects.
