Mari Sonoda heads to Keitaro's cleaning shop with his nephew Jotaro and meets Naoya and Masato, who have returned. Meanwhile, Smart Brain, which has become a governmental organization, has transformed into a company aiming to annihilate all Orphnoch life, and this operation is led by Kitazaki.

Four years after an alternate ending to the series, the Undead has been re-released and Glaive reveals himself to be the Albino Joker. The four Riders are again united, intent on saving Amane and stopping the Albino Joker.

Minoru is diagnosed with cancer and undergoes surgery, but it is soon discovered that the cancer has spread to his lymphatic system and he is prescribed anti-cancer drugs. He is soon plagued by severe side effects such as vomiting, skin rash, and hematuria, and begins to distrust hospitals.

A murder has happened in an isolated pension. Igarashi Ikki, together with his devil partner Vice, and Kadota Hiromi become detectives to solve the mystery.

When Takeru and his friends celebrate Kanon's 15th birthday, the sky suddenly changes its color. They investigate this phenomenon and discover a giant Eyecon in the sky emitting a strange light, which sucked them in.

A time loop created by the Shocker resets whenever someone dies, but whenever the loop resets Shocker itself somehow grows stronger and stronger. The Riders must decide whether to keep on fighting a useless battle or end the loops for good by sacrificing one of their own.
